Required and Necessary Forms The summer tours are a memorable part of the youth exchange experience – and often the highlight. You will find the forms that are required to make a reservation on this page. The forms are listed in two different ways to make it convenient for you to obtain: Adobe Acrobat or PDF (see the special note below under #1 and #2); or Microsoft Word. All students making a reservation must complete:
1) RESERVATION FORM (available below – PLEASE NOTE that the PDF Adobe Acrobat reservation form is interactive which means you should fill in the requested information BEFORE you print it. This will eliminate misreading the information that you provide.)
2) PARENTAL CONSENT FORM (available below - Parents of student may download the consent form in their home country to save time in mailing or faxing the document to them.)
3) RULES AGREEMENT FORM (available below)
Acceptance on a tour is
on a first come basis,
so be sure to sign up
early. In some past
years, the tours have
been filled to capacity
by December 31.
Space on a tour
can not be saved for any
student until ALL the
required documentation
has been submitted and
accepted by the tour
staff
